- README: an ExplicitHasMorePolicy row and a PageFetcher.withSignal example under
  "Deciding where the data ends", a short-last-page custom-policy example, and the
  roadmap item ticked.
- APPENDIX: a decision record for the open PaginationEndPolicy contract, the
  PageFetcher building-block + erased Object? signal, the library-owned signal
  lifecycle, and the end-signal-vs-cursor-driven scope line (+ TOC/anchor).
