refactor(pagination)!: PageFetcher/SearchPageFetcher building-blocks

Turn the fetcher typedefs into small callable classes, so a .withSignal constructor
can carry an end signal (hasMore / cursor) alongside the items in the follow-up,
without forcing that shape on the common path. Behaviour-preserving; completes the
reshape begun in the previous commit (#1).

Migrate every call site to wrap its function (tests, example, benchmark, README).

BREAKING CHANGE: fetchPage/searchFetchPage now take PageFetcher/SearchPageFetcher
instead of a bare function. Wrap existing fetchers:
fetchPage: PageFetcher((pageIndex, pageSize) => ...).
