pin and document schemaless :map decoding via type/2

the "schemaless :map per-field type info" work-queue item closes
with zero new code: Ecto's own type/2 annotation is the per-field
opt-in, and it already decodes JSON through the adapter's loaders on
schemaless queries — whole columns, JSON paths, and inside select
maps, all pinned. An always-decoding custom type is structurally
impossible (untyped select expressions have no Ecto load hook — the
same wall as the boolean-coercion finding); the moduledoc's JSON
section now tells the schemaless story.
