repo-level observability: txn_state, connection_stats, hooks config

XqliteEcto3.txn_state/2 and connection_stats/1 observe a pooled
connection through the pool; the new `hooks:` repo config installs
xqlite's update/WAL/commit/rollback/progress hooks on every pooled
connection at connect time, delivering messages to registered
process names (unresolvable names and unknown kinds are structured
connect errors). Adapter README roadmap item 1 delivered.

discovered and documented while pinning: with_xqlite/3 (and thus
these wrappers) needs its own checkout — calling it inside
Repo.transaction/2 on a plain pool queues behind the transaction's
connection (deadlocks at pool_size 1); Sandbox ownership reuses the
caller's connection so nesting is fine there. Caveat added to
with_xqlite's docs too.
