add XqliteEcto3.Telemetry.OpenTelemetry attribute mapping

the adapter-side mirror of Xqlite.Telemetry.OpenTelemetry: a pure,
dependency-free translation from [:xqlite_ecto3, ...] events to
OpenTelemetry's stable database semantic conventions — query text
from callback query structs (iodata flattened) or statement-cache
sql, db.namespace from connect's database, error.type from the
structured reasons. Spec sources cited per name in the module docs;
