[docs] Add DisaggregatedSet placement policy KEP (#906)

* add kep for DS placement policy

* Update and tweak KEP

* Update kep to only have slice and topology exclusive placements instead of the 3 settings for each

* Clarify placementPolicy update semantics

Address review: make explicit that changing spec.placementPolicy never
triggers a rollout and never patches existing LeaderWorkerSets or pods.
The policy is excluded from the revision hash and injected only at
LeaderWorkerSet creation, so it applies to new slices immediately and
to existing slices on their next rollout.
