statement-cache hit/miss/evicted telemetry

[:xqlite_ecto3, :statement_cache, :hit | :miss | :evicted] events
from the cached checkout and eviction paths, compile-flagged like
the rest of the adapter's telemetry. cached_count (size before the
event's action) plus the statement's SQL — enough to size
statement_cache_size from production traffic. :miss also covers
statements that then fall back to the uncached path.
