url: repo config works out of the box; inline sobelow marks

Ecto's generic URL parsing raises on sqlite:// URLs (demands a host
and single-segment path) and runs after the repo's init/2 — so the
adapter now overrides __before_compile__ (defoverridable in
ecto_sql) to inject a default init/2 into repos that don't define
one, popping :url and merging parse_url!/1 before Ecto sees it.
Repos with their own init/2 are untouched; README shows the two
lines they keep. Pinned end-to-end: url-configured repo boots and
its busy_timeout URL param is observable through the pool.

sobelow skips migrate from .sobelow-skips fingerprints to inline
sobelow_skip marks on the six role-inherent functions — the
fingerprints were position-sensitive and broke on every line shift
above them (twice today). Rationale now lives at each code site.
