audit follow-ups: failure pins, shutdown story, limitations

three verifications from the expectations audit, now test-pinned:
connecting to a missing parent directory surfaces
{:cannot_open_database, path, code, reason} with the exact path;
insert_all from a query source with on_conflict upserts correctly
(the old pre-placeholder-numbering edge does not reproduce); WAL
checkpoint-and-remove on last close is pinned xqlite-side. README
gains the shutdown paragraph and three Known-limitations entries
(materialized views, partitioning, network/replication ecosystem
pointers). Stale sobelow fingerprint pruned.
