require ecto_sql 3.14; drop the pre-3.14 compat surface

user ruling: one supported stack, tested exactly as claimed. Floor
moves ~> 3.12 → ~> 3.14 (transitively decimal 3.x — announcement
honesty ledger carries the adopter cost). modifiers_expr reads the
struct field directly again (the apply/3 checker-opacity and the
Map.put test-injection shim are gone); the fresh-resolve CI lane is
reframed as early warning for future ecto_sql releases. Sobelow
skip fingerprints refreshed (they are position-sensitive; the
modifiers cleanup shifted a skipped finding by one line).
