feat: rank UserPromptSubmit KB context by relevance and recency (#177)

- store.QueryDocuments' non-FTS listing branch now orders by
  updated_at DESC, id DESC (was rowid/insertion order) so any caller
  using it as a plain recency listing gets a meaningful order
- store.KeywordSearch now passes per-column bm25() weights (title 3x,
  content 1x, tags 2x) so title/tag matches outrank body-only matches
- the UserPromptSubmit hook's resume-intent branch now prefers a
  relevance-ranked KeywordSearch when the prompt tokenizes to at least
  one usable term, falling back to the recency-ordered list otherwise
