docs: noun-first CLI surface, server command, backlog writes (OU-327) (#174)

- README: rewrite browse/register sections to the noun-first CLI
  (project/backlog/kb/plan/roadmap/edges), document backlog
  create/update/delete and `ouroboros server` flags
- README: fix `claude mcp add` registration example to pass `server`
  via the `--` separator (bare invocation is help, not the MCP server)
- CLAUDE.md: drop stale query/ls references, name the real noun
  groups, point CLI-only-ops prose at `--help`/wiki instead of
  restating it
- plugin/README.md: drop the stale "10 tools" count
- plugin.json: mcpServers instructions now list roadmap as a write tool
- roadmap.go: reword a stale `ls items --priority` comment
