refactor(cli): dissolve ls command group and remove orphaned query command (OU-331) (#173)

- relocate printTable/printJSON/formatItemDetail/formatKBDetail (ls_format.go) to render.go, dropping dead formatPlanDetail
- relocate runLSEdges (ls_edges.go) to edges_read.go, backing edges_group.go's `edges list`
- relocate newTestDB test helper (query_test.go) to testutil_test.go
- delete ls.go/ls_backlog.go/ls_kb.go/ls_plans.go/ls_projects.go/ls_edges.go/ls_format.go/query.go and their orphaned tests
- reword comments that referenced the now-deleted `ls`/`query` commands
- update the served MCP instructions text (and its testdata fixture) from `ls edges` to `edges list`
