feat(cli): port subagent-stop hook to native Go (#143)

- register hooks.SubagentStop against a new hookHandleSubagentStop,
  a byte-faithful port of plugin/scripts/subagent-stop.js: opt-in
  fenced ```kb block persist only, no decision-language nudge ever
  (OU-222/OU-254 — a subagent's final message is its return value to
  the orchestrator; blocking it would destroy the report)
- extract persistKbBlock/parseKbBlockEntries out of
  claude_hooks_stop.go into a shared internal/cli/hook_kb_persist.go,
  extended with an extraMeta param so subagent-stop can stamp
  agent_id/agent_type onto persisted kb entries
- add isSkippedAgentType to hook_util.go (Explore/knowledge-explorer/
  backlog-manager, namespace-tail aware)
- flip hooks.json's SubagentStop command to the native binary; keep
  subagent-stop.js on disk as inert fallback (removed in OU-264)
