chore: consolidate PR — fix regressions, clean design-doc refs, add design doc

Consolidation pass before MBI-007 merge:

Fixes:
- Restore DOCS_HOME_URL import in __main__.py abort message (was
  printing literal f-string text instead of the URL)
- Fix _run_repl exception handling: ResponseError → continue (retry),
  proper ordering (subclasses before Exception), restore original
  retry-on-response-error behavior lost during cleanup
- Fix session.py typo (stray closing paren in comment)
- Export SimpleContextManager from top-level yoker package (oversight)
- Clean stale BasicContextManager references in context/ docstrings

Design-doc reference sweep:
- Remove all Decision/D1-D10/PR #43/Clarification/Phase/§/MBI-007
  cross-references from src/ and tests/ (42 files cleaned)
- References retained only in analysis/, DEVELOPMENT.md, reporting/

Design document for follow-up PR:
- analysis/context-manager-refactor-design.md: full design for
  ContextManager Protocol + BaseContextManager + ContextManagerWrapper
  + Persisted wrapper, with void-return bulk-rewrite change reporting,
  migration plan, and acceptance criteria

1575 tests pass, lint/typecheck clean.

🤖 Implemented together with a coding agent.
