Handle duplicate keys in CoalescingBulkLoader

Two loads of the same key in one batch made Collectors.toMap throw
IllegalStateException, and the hookless subscribe let onError cancel the
subscription — bricking the loader so every later load hung. Collect the
futures per key with groupingBy and complete them all (a merge that kept
one future would silently strand the other). Add an error consumer so a
terminated pipeline is logged rather than swallowed by the global hook.
