Switch generated code to the kratoserrors package

Update the template and code to emit kratoserrors, matching kratos-errors v0.0.3.
Drop the obsolete metadata example and shorten the long enum value to INVALID_PARAM.
Fix the errgenkratos package doc name, refresh both README docs, and clean up comments.
