io: cache the monotonic clock across loop iterations (perf)

now_ns() made a vdso clock_gettime call per invocation, and deadline
arithmetic invokes it from every callback — ~3% of data-path CPU in
the 73k req/s flamegraph. A first attempt caching once per iteration
measured no better: the loop iterates about as often as the clock was
read. The clock now refreshes after any blocking wait (so post-block
time is correct) and at most every 256 busy iterations. Busy
iterations run in microseconds, so staleness stays far below the
second-granularity deadlines and the one-second timer tick.
