fix: align empty-ingress + body-parse handling; parameterize test query

Addresses review comments:
- streaming-key-ttl-checker: skip removeIngress('') for empty-ingress Cast 2.0
  rows so removeAccess still runs (mirrors streaming-ttl-checker); otherwise the
  row stayed active and re-errored every tick.
- watcher-token-handler: wrap request.json() in try/catch -> 400, aligning with
  remove-scene-admin-handler (the schema middleware already guards this; it is
  defense-in-depth if the route is ever wired without it).
- ban-status concurrent-ban test: use the SQL template tag instead of string
  interpolation in the verification query.
