docs(rippling): update reach-filter comments now rippling is live

Drop the 'inert until the reach engine populates rippling_reach' / 'ships in PR A'
/ 'pre-engine' framing from FilterReachBlocked and the reach-gated count/metrics
comments - the reach engine is live and populating rippling_reach. Keeps the
accurate note that the filter is a no-op for posts with no reach row (non-rippled
posts) and when the table is absent. Comment-only, no behaviour change.
