feat: bb_vcore_wd FAULT_HOLD on OC fault — don't recover into a latching over-current (TA-435) (#479)

- generic bool oc_fault input (keeps bb_power_health regulator-agnostic)
- BB_VCORE_WD_FAULT_HOLD latching action
- fault_held in state + is_held/clear_hold accessors for consumer NVS persistence
- UNIT_OFF/rail-off is not a fault, only oc_fault holds
- over-temp deferred to TA-189
