flyscan: prefer AD-file timestamps in post-stub Step 2

Step 2 (/entry/positions per-frame correlation block) now uses
pair_frames_to_positions_from_ad_file when the external AD HDF1
file is openable, falling back to pair_frames_to_positions (CA
monitor source) when it isn't.

The AD-file source eliminates the image-number gaps from CA
monitor coalescing: the IOC writes one row per acquired frame
into the file, so every frame is recoverable.

Loop B (was previously between steps 2 and 3) now runs once before
step 2 and gates both steps -- both need the external file
reachable.  Step 3 consults the same external_file_openable flag
instead of re-running the open check.

Real-data check on scan 35: AD-file path returns 102 in-scan
frames, CA-monitor fallback returns 67.  35 frames recovered.
