ACP message polish and config option switching (#2098)

* feat: polish ACP messages and model switching

Surface ACP session updates in transcripts and route ACP model/thinking changes through session config options without restarting active ACP queries.

* fix: harden ACP config switching

Rollback failed ACP model switches and keep dynamic config/model caches plus context usage estimates current.

* fix: refresh ACP runtime state

Handle ACP usage updates as context capacity, refresh global model cache after config discovery, map thinking controls to advertised values, and preserve transcript order around synthetic updates.

* fix: preserve ACP runtime caches

Keep ACP usage totals separate from local estimates, honor configured context windows, clear stale dynamic models, preserve discovered models in global cache, and restore provider session IDs on rollback.

* fix: sync ACP config option updates

Apply persisted ACP model choices before prompting and propagate set_config_option responses into provider/global caches.

* fix: guard ACP model reconciliation

Skip placeholder model application, preserve global fallback models, and carry ACP context estimates across turns.

* fix: align ACP context and thinking state

Count prompts in fallback context estimates, allow reported usage decreases, reconcile stored thinking level, and prefer ACP-reported capacity.

* fix: sync ACP reported runtime state

Use ACP-provided thinking choices, sync agent-side model updates, carry context estimates per turn, and keep reported context usage out of API token totals.
