v1.31.10: sex-stratified reproductive tissue tiers (closes #286) (#288)

Split the coarse somatic-vs-reproductive tissue classification into three
reproductive tiers keyed on what actually makes a CTA target safe:

- reproductive (privileged): testis, epididymis, placenta, embryo —
  immune-privileged / fetally tolerized, CTA-safe regardless of patient sex.
- reproductive_female: breast, mammary gland, lactating breast, ovary, uterus,
  cervix, endometrium, fallopian tube, vagina.
- reproductive_male: prostate, seminal vesicle.

The female/male tiers present antigen normally, so they are a safety signal
only in patients who have the tissue — patient sex becomes an extra safety
stratum (applied downstream; hitlist tags only the tissue's sex). By default
they are pulled out of src_healthy_tissue (no longer a hard somatic safety
signal), fixing #286 where healthy breast was mislabeled THE SAFETY SIGNAL and
disagreed with downstream CTA tooling.

New flags: src_healthy_reproductive_female, src_healthy_reproductive_male
(wired into aggregate.py, samples.py, report.py). testis stays privileged
(not male) because the blood-testis barrier, not sex, is its safety basis.
