Log DESIRED_STATE rotation payload outcomes (#1112)

Replace silent \let _ =\ discard in \handle_desired_state\ with
explicit match that logs:
- \info\ on successful rotation application
- \warn\ with rejection reason on failure

This aids debugging when a \otation_payload\ is repeatedly relayed
but the gateway silently fails to apply it (e.g., epoch-bound
decryption failure after the rotation already completed).

Update \gateway-design.md\ §23.12 to document the new logging
behaviour.
