docs: adopt Code of Conduct + document contribution licensing and license-header rule (#509)

Closes the three OpenSSF Best Practices `contribution_requirements` gaps
that CONTRIBUTING.md did not yet spell out:

- **Code of Conduct.** Adopt Contributor Covenant v2.1 verbatim in
  CODE_OF_CONDUCT.md. The enforcement contact reuses the private
  GitHub security-advisory intake already documented in SECURITY.md,
  avoiding a new mailbox / exposed personal address.
- **Licensing of contributions.** State the Apache-2.0 inbound = outbound
  convention explicitly so contributors (and OpenSSF reviewers) don't
  have to infer it from GitHub ToS alone. No CLA / DCO sign-off
  required, matching how the project has operated so far.
- **License headers on Rust source.** Document the existing
  `header-check` CI job and `scripts/check-ts-header.py --fix`
  workflow, including the gotcha that the pre-commit hook does not
  run the header check (a real source of confusion for new
  contributors who only run the cargo/clippy hook).

Single CONTRIBUTING.md URL now transitively covers OpenSSF's
`code_of_conduct` and reinforces `floss_license_osi` alongside
`contribution_requirements`. No code changes; `python3
scripts/check-ts-header.py` passes (264 files OK).
