Testing rule: fix coverage gaps on every touched file (fix-on-touch) (#4350)

Tightens `.claude/rules/testing.md` per-file coverage guidance:
every PR is a chance to lever coverage upward. If a touched file is
below 100%, fix the gap in the same PR — even if the uncovered
lines were already missed on `main` before the edit.

The previous wording carved out an exception for pre-existing gaps
("not your bug to fix in this PR — but surface it in the PR
description"). In practice that meant the same gap survived every
future PR that touched the file until someone finally signed up to
fix it. Fix-on-touch is the only sustainable policy.
