docs: trim persist skill step 6; close stale OU-135/OU-136 (#69)

- remove the hook-internals aside from persist SKILL.md step 6;
  model only needs the block format, not the scanner's role
- OU-135 and OU-136 were already implemented (pre-compact.js
  queries ouroboros on zero blocks; tests cover both the tool
  path and the heuristic fallback). Closing as done.
- closes OU-134, OU-135, OU-136
