fix: tighten backlog-manager subagent prompt (OU-157) (#68)

- always pass `limit: 50` on item list ops; paginate or narrow filters
  instead of dumping the full backlog (root cause of tangents on a
  50+ item backlog)
- stop re-confirming work the parent prompt already authorized; item
  create is not destructive and should execute directly even for batches
- closes OU-157
