fix: fire stop.js nudge; dedupe hook script logic into lib.js (OU-170) (#70)

- stop.js defined ALREADY_PERSISTED_PATTERNS and DECISION_PATTERNS but
  never matched against them; main-context turns with decision language
  slipped through silently (OU-170)
- extract shared logic into plugin/scripts/lib.js:
  - ALREADY_PERSISTED_PATTERNS, DECISION_PATTERNS constants
  - checkNudgePatterns(message, ctx) — tier-2/tier-1 dispatch
  - persistKbBlock(message, ctx) — kb block extract+validate+put
  - readLastMainAssistantText(transcriptPath) — moved from stop.js
  - queryKb(project, opts) — single CLI query wrapper
- stop.js and subagent-stop.js now share the kb-persist and nudge paths
- post-edit-check.js, user-prompt-context.js, pre-compact.js all
  delegate their query calls to queryKb
- update the two existing tier tests that documented broken stop.js
  behavior to assert exit 2 + decision:block stdout
- closes OU-170
