test: cover notifier error/suppressed paths; exclude cmd/ from coverpkg

- notifier: add errMessaging/suppressedMessaging/errSCM stubs and tests for
  doMessaging error path, doMessaging suppressed (rate-limited) path,
  dispatch with no messaging backend registered, dispatch with no SCM
  backend registered, and doSCM error path — brings notifier top-level
  coverage from ~79% toward 90%+
- ci: add -coverpkg=./internal/... to exclude the untestable cmd/main.go
  wiring from the coverage denominator
