test: add DB integration tests and Dial test to push coverage above 90%

- interceptor: TestInterceptorWithDB covers entity/operation/event DB write
  goroutines and parent resolution; TestInterceptorWithDBAndNotifier covers
  the icp.notifier.Send() path; TestInterceptorMultipleUnknownParents covers
  multi-parent resolution miss path — all skip when TEST_DB_URL is absent
- receiver: TestDial covers the Dial() happy path (grpc.NewClient is lazy)
- api: monitorBins success + nil-store + store-error + clamp + parse-error
  tests; entity graph/operations/silences tests with noopMetrics
