chore: use pnpm publish directly for releases with provenance

Replace 'changeset publish' with 'pnpm -r publish' so the release path
is purely pnpm-driven. pnpm v11 supports --provenance natively (ties
the published artifacts to the GitHub Actions build via OIDC, which
the workflow already grants via id-token: write). pnpm -r publish
skips packages whose versions are already on the registry, which is
the same behavior changeset publish provided.
