vm: mark include/require/eval as slot-unsafe in their callers

include(), require(), include_once, require_once, and eval() execute
PHP code in the caller's scope. The bytecode they end up running (or
the AST-delegated execution) reaches for caller locals via
ctx.Parent(1)'s FuncContext hashtable. If the surrounding body was
slot-only, locals like `\$autoloading = true` set inside the caller
never reached the hashtable and the included file saw nothing.

Fixes autoload/bug63741 and any other test that relies on caller-
scoped variables visible through include/eval.
