vm: harden by-ref detection for NoticeRef/PreferRef and add missing static entries

callableHasByRef now treats NoticeRef and PreferRef the same as Ref —
all three need ref-binding semantics that the VM's value-passing call
protocol can't provide, so the emitter must AST-delegate. The static
ByRefBuiltins map gains entries from a full audit of ExtFunctionArg
declarations (mb_convert_variables, extract, exec/passthru/system,
fscanf, sscanf, getimagesize, str_(i)replace, preg_replace*, etc.) so
emit-time queries without a Global still catch them.
