feat(ai): add `arc ai usage` cmd for tracking (#14)

### Description

Adds `arc ai usage` to pull quota usage from Claude Code (Anthropic
OAuth), Codex (`codex app-server` RPC), and Cursor (SQLite +
dashboard/REST APIs) in parallel, with optional TTL cache and structured
JSON via the root `-j` flag.

## Added

- `arc ai usage` plus `internal/ai` providers, parallel runner
(`errgroup`), file cache, and terminal presentation in
`internal/ai/presentation`
- `docs/ai_usage.md` for credentials, call budget, cache, and human vs
JSON output

## Updated

- `go.mod` / `go.sum` (`golang.org/x/sync`, `modernc.org/sqlite`) and
`internal/output` (`SectionAccent` for section headers)
