feat: add `arc ai pricing` cmd (#26)

## Description

Adds `arc ai pricing`, a command that fetches a current model pricing
table into a local cache so `arc ai tokens` cost estimates stay accurate
without an `arc` release. Pricing is resolved from a hand-edited
override over the fetched cache over built-in defaults, keeping `arc ai
tokens` fully offline.

### Added

- `arc ai pricing` command (`--source`, `--dry-run`, `-j`) that fetches
LiteLLM's table into `~/.cache/arc/ai-pricing.json`.
- `LayeredPricer` plus pricing fetch/cache/override loaders in
`internal/ai`, with tests.
- `docs/ai_pricing.md` documenting the layering and override file
format.

### Updated

- `arc ai tokens` now prices via the layered pricer instead of the
static table.
- README, AGENTS.md, and `docs/ai_tokens.md` to reference the new
command and layered pricing.
