refactor(asic): share telemetry register map between BM1368 and BM1370 (#164)

- move REG_{ERROR,DOMAIN_0..3,TOTAL}_COUNT from bm1370.h to asic_internal.h as ASIC_REG_*
- rename the ten call sites in asic_task.c
- BM1368 and BM1370 use identical offsets (0x4C, 0x88-0x8B, 0x8C) per AxeOS REGISTER_MAP,
  so the existing poll loop now yields per-chip telemetry on bitaxe-403 with no extra gating
- verified on hardware: bitaxe-403 reports chip 0 total_ghs and hw_err cleanly,
  bitaxe-650 dual-chip telemetry unaffected
