feat: per-chip instantaneous hw_err_pct on /api/stats.asic_chips[] (#162)

expose the 5s-delta-based error rate per chip directly, matching the
shape of AxeOS's errorPercentage (point sample, not long-window average).
helps close the TA-198 measurement-semantics question: our
asic_hw_error_pct_10m was a 10-min progressive-blend while AxeOS's
number is a recent delta, making the raw 1.7% vs 0% comparison misleading.

this doesn't close the silicon-bin gap where our chip 0 reports ~2.5%
while AxeOS's worst chip sits around 1.3%. that residual appears to be
intrinsic to the specific BM1370 dies on each unit.
