fix: program TPS546 switching freq + OC fault config + clear faults at init (#158)

bitaxe boards shipped with the TPS546 OTP default 400 kHz switching
frequency. AxeOS programs 650 kHz on every boot; we were relying on OTP.
at 400 kHz the inductor ripple is ~62% larger, pumping VDD noise into
the BM1370 supply and raising the silicon hash error rate. observed
~2% HW error on TM vs ~0.9% on AxeOS at identical 490/1100 config.

also program IOUT_OC_FAULT_LIMIT=30A and IOUT_OC_FAULT_RESPONSE=0xC0
(shutdown, no retry) so OCP behavior is deterministic rather than
unit-specific OTP. call CLEAR_FAULTS before OPERATION=ON so stale
latched faults from a prior soft-reset don't glitch VDD during ASIC
bring-up.

four new PMBus writes in tps546_init alongside the existing VOUT_COMMAND
and OPERATION. SLINEAR11 encoder handles the two small-int operands
(650, 30) with exponent 0.
