* docs: research PR merging & completion actions across workflows

Analyzes where PR merge decisions are made in Coding, Coding-with-QA,
Research, Review-Only, and Plan & Decompose workflows. Identifies three
gaps, most notably the Coding-with-QA workflow prompts the QA agent to
run `gh pr merge --squash` directly regardless of space autonomy level.

* docs: add failure-modes + consolidation proposal to research

Appends §10 (Failure Modes of the Completion-Action Pipeline) cataloging
the blocked-task failure surface, false-negative block problem, and
partial-failure hazard. Appends §11 (Proposal: Consolidate into Gates +
Nodes) with per-action migration table, minimum-viable-fix fallback,
and the rationale for treating gaps #1/#2/#3 as one removal rather than
three patches.

* docs: update §11 — Task Agent as merge executor (not new node)

Replace the "add a merge node to each workflow" proposal with the
architectural decision: the Task Agent handles post-workflow PR
merging directly, using request_human_input for levels 1-3 and
auto-executing at levels 4-5. Completion actions are removed
entirely. Gaps #1, #2, and #3 all disappear with the mechanism.
