v1.12.0: aggregate_per_pmhc_with_refs — per-pMHC aggregation with provenance (#91)

Adds a third canonical aggregation to hitlist.aggregate that sits between
aggregate_per_peptide (per-peptide, keeps provenance, loses allele
granularity) and aggregate_per_pmhc (per-(peptide, allele), lean, loses
provenance). Keeps allele granularity AND surfaces the provenance columns
reviewers want on a pMHC row: reference count, PMID list, distinct
tissues / diseases / cell lines, cancer / healthy-tissue source flags,
mono-allelic hit count.

Output columns follow hitlist's ms_pmhc_* prefix convention. Only peptide
+ mhc_restriction are required; all other columns are conditional on the
input so both the cached-observations fast path and the raw-scan slow
path produce valid output. PMIDs sort numerically via a new
_join_unique_numeric helper.

Upstreams tsarina v0.9.0's local _aggregate_refs. tsarina will drop the
duplicate in a follow-up PR once this ships.

Closes #88. 227 passed, 1 skipped.
