refactor(http_server,ota_pull): opaque bb_http_handle_t in public API (#18)

Replace httpd_handle_t in http_server.h and ota_pull.h with an opaque
bb_http_handle_t (typedef void *) so consumer headers no longer need
esp_http_server.h. Internal ESP-IDF implementation is unchanged; casts
at the public boundary are no-ops since httpd_handle_t is also void *.
