doc: add update-flow sketch (fast path + dirty-set worker) to Phase 2

Replace the "implementation freedom" punt with a brief two-paragraph
spec: a loader-side fast path that emits additive evidence in the same
transaction as the raw nanopub write (so simple member additions are
visible in the same 2s load cycle), backed by a dirty-set worker that
runs full recomputes for transitive effects, trust-state flips, and
invalidations. Pin one materialization-contract constraint:
deterministic IRIs for RoleAssignment / Evidence so fast-path and
recompute writes are idempotent.
