fix: drop getProject bare git fallback (OU-115) (#48)

Removed the bare git rev-parse fallback in getProject() and the skipGit parameter in resolveProject(). The fallback always resolved to the plugin's own repo, never the intended project.

- Delete getProject() function from lib.js (lines 18-27)
- Remove git rev-parse fallback from resolveProject() (lines 173-188)
- Remove skipGit parameter and update priority comments
- Update all hook scripts: stop.js, subagent-stop.js, subagent-start.js, post-commit-nudge.js, post-edit-check.js
- Remove getProject() from module.exports
- Update test fixtures: remove skipGit parameter, add workspace/git setup for integration tests
- Update comment in user-prompt-context.js to reflect new priority chain

Scripts now fail silently when project cannot be determined via cwd or hints (message/transcript), instead of silently resolving to the plugin repo.
