fix: correct Stop/SubagentStop hook output contract (#47)

Tier-1/2 nudges were emitted via console.error, bypassing agent context injection. Fix by:
- subagent-stop.js: tier-1/2 paths now emit decision:block JSON on stdout with exit 2
- stop.js: tier-1/2 paths removed entirely (auto-persist KB, no nudge needed)
- Tests updated to assert new contract (stdout JSON, exit 2 for subagent; clean exit 0 for stop)
