fix: scope post-commit-nudge cooldown per-project (OU-116) (#49)

- replace global /tmp/.ouroboros-commit-nudge with per-project variant /tmp/.ouroboros-commit-nudge-{project}
- sanitize project names to replace path-unsafe characters with hyphens
- ensures cooldown in project A doesn't suppress nudge in project B
- add two new tests: per-project scoping + sanitization verification
