Fix healthy override: add explicit branch in classify_ms_row

The YAML defined override: healthy for 3 studies (PMIDs 33858848,
27862975, 36589698) but classify_ms_row had no branch for it,
so the override fell through to default structured-field logic.

Now forces is_cancer=False and is_healthy_donor=True when the
effective override is "healthy", regardless of IEDB process_type
or disease fields.

Adds regression tests for both study-level and conditional-rule
healthy overrides.
