* fix: update ingest help text from CBOR to BPF ELF

The `program ingest` subcommand help said "Ingest a CBOR program image"
but release builds only accept ELF files. Update the clap doc-comment
to "Ingest a BPF ELF program" and the FILE argument to "Path to the
BPF ELF object file". Also update gateway-design.md CLI synopsis to
match.

* docs: drop "when enabled" per review feedback

The CBOR-in-debug note implied a separate feature flag, but the gate
is just `cfg!(debug_assertions)`. Simplify to "only accepted in
debug/development builds".
