feat: wire SSM transport for private-subnet cluster nodes (#727)

Connect SSMTransport to the cluster provisioning flow so that nodes
without public IPs (private subnets) automatically use AWS SSM port
forwarding for SSH connections.

Extract buildClusterNodeInfoList() from runMultinodeProvision() to make
the node-building logic testable. The function now populates InstanceID
from NodeStatus and conditionally creates SSMTransport when a node has
no public IP but has a valid instance ID.
